What an NCV test measures

A nerve conduction velocity (NCV) test, also called a nerve conduction study, measures how fast and how strongly electrical signals travel along a nerve. Healthy nerves carry signals quickly. Damaged or compressed nerves carry them slowly or weakly.

It helps identify the cause and level of numbness, tingling, burning pain and weakness in the hands, arms, legs or feet. Common reasons include carpal tunnel syndrome, ulnar nerve entrapment, pinched nerves in the neck or lower back, diabetic and other neuropathies, and Guillain-Barré syndrome.

How the test is done

• Small flat electrodes are stuck on the skin over the nerve and the muscle it supplies.
• A brief, mild electrical pulse is applied to the nerve. It feels like a quick tap or tingle.
• The response is recorded and the speed, size and shape of the signal are measured.
• Several nerves in one or both limbs may be tested. The test usually takes 30 to 60 minutes and is often combined with EMG.

How to prepare

• Bathe on the day of the test and do not apply oil, lotion or cream on the limbs.
• Wear loose clothing that lets the technician reach your arms and legs. Keep the limbs warm.
• Tell us about a pacemaker, defibrillator or any implanted electrical device, and about blood thinners.
• Carry previous reports, MRI scans and your medicine list.

Slow conduction usually suggests damage to the nerve’s insulating covering, while a weak signal suggests loss of nerve fibres. The pattern across different nerves helps your doctor see where the problem lies. Nerve conduction velocity

Nerve conduction velocity (NCV) is a test to see how fast electrical signals move through a nerve. This test is done along with electromyography (EMG) to assess the muscles for abnormalities.

How the Test is Performed
Patches called surface electrodes are placed on the skin over nerves at different spots. Each patch gives off a very mild electrical impulse. This stimulates the nerve.The nerve’s resulting electrical activity is recorded by the other electrodes.

 The distance between electrodes and the time it takes for electrical impulses to travel between electrodes are used to measure the speed of the nerve signals.

Electromyography (recording from needles placed into the muscles) is often done at the same time as this test.

Do I need a doctor's prescription?

Does NCV hurt?

Most people feel only a brief tingling or tapping sensation. It is generally well tolerated, including by children who can sit still.

Why must the limb be warm?

Cold lowers nerve conduction speed, which can give a false abnormal result. A warm limb gives a more reliable measurement.

Is NCV the same as EMG?

No. NCV tests nerves with skin electrodes. EMG tests muscles with a fine needle electrode. They are often performed in the same sitting.

Can I have NCV if I am diabetic?

Yes. NCV is commonly used to detect diabetic neuropathy. Take your usual medicines and meals and tell the technician about your condition.
